Grace Rehab at Syverson Lutheran Home provides short term rehabilitation patients the care necessary to improve their quality of life. We provide both skilled rehabilitation services — including physical therapy, occupational therapy, and speech therapy — and skilled medical services — including, but not limited to, post surgical care, respiratory services, wound care, intravenous therapy and renal disease services.
Our experts take an integrated team approach to your care. This means understanding your needs and therapy requirements on an individual basis, as well as educating you and your family members about your therapies and their expected outcomes. |
Rehabilitation is often a critical component of a resident’s care plan. Our skilled team of nurses, as well as physical, recreational, speech and occupational therapists, evaluate your rehabilitation needs to design an outcome-oriented treatment program.
Our goal is to accelerate healing and improve your quality of life to the highest degree possible. When healing permits you to resume a more normal life, our staff will develop a discharge plan to continue your progress, including necessary in-home evaluations and caregiver training to ensure a smooth transition home. |
