Augusta Health and Rehabilitation selects Grace Lutheran Communities as Management Partner

Augusta, WI – Augusta Health and Rehabilitation has chosen Grace Lutheran Communities to assist in the management of its new skilled nursing facility and SilverLeaf Augusta/Assisted Living.
 
Augusta Health and Rehab is a top-rated facility with a proud history dating back over five decades and generations of families.  The new, state-of-the-art facility opened in 2016 and serves residents with skilled nursing, assisted living and provides rehabilitation for residents as well as out-patients.
 
Augusta Health and Rehabilitation Administrator Jahn Bradley commented, “Partnership with Grace Lutheran Communities complements our proud heritage of service, adding their decades of management experience and purchasing capacity.  We look forward to increased cost savings and efficiency, plus implementation of enhanced training and care techniques, all designed to further enrich our well-known reputation for quality care.  It’s a strong cooperative effort in which both Grace and AH&R can grow, however, those who gain the most are the residents, present and future.”

Grace Lutheran Communities is a non-profit organization specializing in rehabilitation, assisted living, skilled nursing, child care, independent living and adult day services.  Facilities include River Pines Long-Term Concierge Care, Prairie Pointe Short-Term Rehabilitation Suites, Syverson Lutheran Rehab and Health, Grace Edgewood assisted living, Grace Willowbrook assisted living, Grace Woodlands memory care, Grace Barstow and Parkside apartments, Grace School-Age Childcare and Grace Adult Day Services in both Chippewa Falls and Altoona.  The foundation also manages senior care facilities for American Lutheran Communities and First Lutheran Foundation as well as the community owned Pioneer Nursing Home in Prairie Farm, Wisconsin, and Heritage of Elmwood Nursing Home in Elmwood, Wisconsin.
